Consumer Health Data Privacy Policy
This policy explains how Alexandra Somm Fit ("I," "me," "my") handles consumer health data collected through alexandrasomm.com, mainly through the coaching onboarding form (Get Started). It supplements my general Privacy Policy.
Health Data I Collect
- Physical Activity Readiness (PAR-Q) answers, including heart conditions, chest pain, dizziness or loss of consciousness, bone or joint problems, blood pressure or heart medication, and any other reason you shouldn't exercise.
- Existing health conditions, medications, surgeries, and injuries.
- Allergies and food intolerances.
- Body measurements (age, height, weight) and physique goals.
- Fitness level, training history, eating habits, calorie-tracking history, sleep, and weekly schedule.
Why I Collect It and How I Use It
I use this information to assess whether it's safe for you to start training, decide whether I need written clearance from your doctor, design your training and nutrition program, communicate with you, and deliver and adjust your coaching. I don't use it for advertising or profiling, and I never sell it.
Where It Comes From
I collect this information directly from you when you complete the onboarding form or share it during coaching. I collect it only after you check the consent box on the form.
Who I Share It With
I don't sell your health data, and I don't share it with advertisers or data brokers. It is processed on my behalf by the services I use to run my business: Formspree (form delivery), Google (Gmail), Zapier and Notion (organizing inquiries and clients), and, if you become a client, Everfit (client portal). I may also disclose it if required by law.
Your Rights
You have the right to:
- confirm whether I'm collecting, sharing, or selling your health data, and access it, including a list of the third parties and affiliates I share it with;
- withdraw your consent to my collection or use of it at any time; and
- ask me to delete it.
To exercise any of these rights, email alexandrasommfit@gmail.com. I'll respond within 45 days (I may extend this once by another 45 days if needed, and I'll tell you if I do). If I decline your request, you can appeal by replying to my response with the word "appeal," and I'll respond within 45 days. If you're still not satisfied, you can contact the Washington State Attorney General's Office.
How Long I Keep It
If you submit the onboarding form but don't become a client, I keep your answers only for as long as I reasonably need them to follow up with you, and I delete them when they are no longer needed or sooner if you ask. If we work together, I keep your information while we work together and for a reasonable period afterward in case of questions or disputes, and then delete it. You can ask me to delete your health data at any time.
Security
I take reasonable steps to protect your health data, including limiting access to it to me and the services listed above. No method of transmission or storage is completely secure.
